This information is not accessible from DCW in situations that may arise due to an injury, behavior issue, alcohol issue, etc. There is no way to print any of these waivers with a signature. We require signatures on all these documents in our hard copy registration forms.
Also, participants are not required to sign the liability waiver online during annual program registration, this one in particular needs to be required and stored in case of an incident.

The online signature the parent adds after registration is meant to include all questions and forms associated with the online registration process. Your PDF enrollment form can be mapped from the responses parents provide during the process.
