Our summer camps require both immunizations and a well-child statement/physical. Parents usually mail the paper copies. This would eliminate a large amount of wait time for required documents. It would also allow parents to upload behavior plans, signed injury/behavior reports.

This feature already exists. Be sure to enable activity ID 1821 for the family portal (not allowed) role- setup > admins > setup roles.
Once that has been enabled you can setup document types for upload by going to setup > system config > child docs.
