Customer wants a Standard Report that they can print out for licensing needs that will display all of the tracking items and information that they track.
Customer has licensing requirements for all of the information that is captured on their enrollment form - when they print the email for a newly submitted registration it does not have several required fields and the formatting is difficult to read. Data Viewer is unable to show payer, authorized pick up and child information into one printable view, and there is a limit on how many categories can be selected in a Standard Report.
Existing report "Child: Information Sheet w/ Categories (Landscape)" is an excellent baseline for their requirements, but they need to be able to select up to 30 categories of Tracking Item / UDF instead of the 4 that the root report allows users to select. This would allow them to choose all of the TI/UDF categories that they use on their enrollment form.
Further, they also need a signature line and most recent enrollment/withdrawal dates displayed on the report itself.

Customer clarified the request - updated the Aha submission with the details. Essentially they want the same report as the root report because it contains all of the information they need for licensing. Their updated request is to allow users to select up to 30 Tracking Items and/or User Defined Fields (the root report allows for up to 4 choices).
