incident report under standandars

Hi.My daycare is in the state of Virginia and I had a violation this week because the incidents report don't contain all the information that licensing requires. so I wanted to see if there is a way to add that information.But I totally undestand every state is different regulations

The information recorded shall include the following:

1. Date and time of injury;

2. Name of injured child;

3. Type and circumstance of the injury;

4. Caregiver present and action taken;

5. Date and time when parents were notified;

6. Any future action to prevent recurrence of the injury;

7. Caregiver and parent signatures or two caregiver signatures; and

8. Documentation on how the parent was notified.
